HOME OFFICE APPOINTMENT
$25 for the first stamped document and $5 for each additional document stamped thereafter
MOBILE APPOINTMENT
$30-$35 (depending on your area) for the first stamped document and $5 for each additional document stamped thereafter

PRE-APPOINTMENT CHECKLIST
PLEASE REMEMBER TO:
Bring at least one piece of government-issued photo identification; and complete the forms/documents that are to be commissioned, except where your signature is required as your signing must be done in the presence of the commissioner
